The Perry Cook Library offers a library card to any Ohio resident. To apply for the card you will need a completed application, available proof of your current address (utility bill, bank statement, lease agreement) and a photo ID.
A parent or guardian must sign for any patrons under 18 years old.
You library card must be presented every time you check out materials or use the public computers. Your first library card is on us, but replacements are $1.00 each. .
