Perry Cook Memorial Public Library offers free delivery of materials to persons of any age who are residents living within the boundaries of the Northmor School District, if they meet the following criteria: either temporarily or permanently confined to their home or care facility due to illness, injury or disability.
To receive home delivery, please fill out this Homebound Delivery Service Application form or download/fill-in/print the attached Homebound Delivery Service application form and return to the library.
