Perry Cook Memorial Public Library offers free delivery of materials to persons of any age who are residents living within the boundaries of the Northmor School District, if they meet the following criteria: either temporarily or permanently confined to their home or care facility due to illness, injury or disability.

If you have any questions about our free homebound service or would like to receive this service, please call 419-362-7181 or print and fill out the attached application form and mail it to the library. Application forms are also available for pickup at the library.

Policy

Perry Cook Memorial Public Library will provide delivery and retrieval of library materials free of charge to and from eligible patrons who cannot drive due to age, injury, illness or other restriction.  Patrons who need this service on a temporary basis also qualify.  Outreach patrons must have a library card and reside within the Northmor Local School District boundaries (the Perry Cook Memorial Public Library service area).  Deliveries are limited to library materials only.
